Immunizations (shots) are provided free for eligible children ages 6 weeks - 18 years through the Vaccines for Children program.

Adults (age 19 years and up) will be charged a fee. Please contact the Santa Rosa Department of Health for available adult vaccines.

Immunizations, and immunization related needs, are offered by appointment only.  To schedule an appointment, call 850-983-5200 and select option 3.

Florida School Requirements

The Immunization Guidelines (PDF-54KB) for the State of Florida cover entry, attendance or transfer to preschools, public/non-public schools (K-12), licensed daycare facilities, and family day care homes. The school the child attends will have on file a Florida Certification of Immunization (also referred to as a "Form 680").
