The Florida Department of Health in Santa Rosa County (DOH-Santa Rosa) provides culturally sensitive services to meet the needs of our diverse community and to protect the health and well-being of Santa Rosa County residents.

This supportive environment assists our patients in taking an active role in their personal health promotion and health maintenance. We offer a variety of health education initiatives and services.

Clinical services offered at DOH-Santa Rosa include:

  • Family planning care
  • Immunizations
  • Sexually transmitted disease (STD) evaluation and treatment
  • The Florida Breast and Cervical Cancer Program
  • HIV, PrEP and ADAP services
  • We Care
  • Breast and cervical screenings
  • Pregnancy testing
  • School and sports physicals

Our medical services are designed to give affordable access to medical care for insured, uninsured and underinsured clients.

Patients providing qualifying income information may be eligible for reduced rates.

Additional Programs 

The Florida Breast and Cervical Cancer Program
Assists uninsured women 50-64 that qualify financially for services. Services include a pap smear and mammogram.

Prior to your visit, we have made available online some of the forms you may need.  Download the form, print it, fill it out and bring it to the health department on your next visit.

Immunization Records Transfer Form 680 Request (PDF-60KB) - a record of immunizations.  This is required for school or daycare entry.
