We offer a variety of healthcare services to meet the needs of the people in the community. Some services such as physical exams and family planning must be scheduled on days that a doctor, nurse practitioner, or nurse is available. Although appointments are not required for all clinics, we encourage appointments so we can see you as quickly as possible.
Immunizations are FREE for eligible children through the age of 18 through the Vaccines for Children Program. Payment is necessary for adult vaccines per approved fee schedule.
Clinic Services are provided on a sliding fee scale basis. Clinic fees are based on income and family size at the initial visit and are updated annually or as income and family size changes. We accept Medicaid, and for those who are uninsured or prefer to pay out-of-pocket, we accept cash, check, Visa and Mastercard.
Language translation services are available for clients who do not speak English.

Form 680 Request (PDF-64KB) - a record of immunizations. This is required for school or daycare entry.
