Florida Department of Health Santa Rosa Midway Location to Open 

Milton, FL – The Florida Department of Health in Santa Rosa County announces the reopening of its Midway Clinic. The clinic, which has been closed due to heavy damage caused by the April 2014 flood, will reopen on Tuesday, November 3. The Midway facility is located at 5840 Gulf Breeze Parkway in Gulf Breeze.

Immunizations will be offered on a walk-in basis from 8:00 a.m. to 11:30 a.m. and 1:00 p.m. to 3:00 p.m. Family Planning services and exams for sexually transmitted diseases (STD) will be available by appointment. Clients may schedule appointments by calling the Milton location at 850-983-5200, ext. 107.

The health department also continues to offer WIC services at the Midway location. For more information on WIC eligibility, clients may call the WIC office in Milton at 850-983-5250.
