TDAP Clinic at Midway Location

7th Grade TDAP Clinic July 30 

Milton, FL – A special immunization clinic for students entering 7th grade in the coming school year will be held July 30 at the Florida Department of Health in Santa Rosa County Midway facility, 5840 Gulf Breeze Parkway in Gulf Breeze. The Midway location has been closed for repairs since it was heavily damaged by a flood in April 2014. The special clinic is an effort to provide an opportunity for parents of 7th grade students to get their child the Tdap (Tetanus-Diphtheria-Pertussis) booster required for all students entering 7th grade for the first time.

Shots will be provided on a walk-in basis from 8:00 a.m. to 11:30 a.m. and 1:00 p.m. to 3:00 p.m. Children must be accompanied by a parent or legal guardian. Documentation of guardianship is required. Parents are asked to bring a copy of their child’s immunization record. A copy of the Florida Certificate of Immunization (Form DH 680) will be provided. Only the Tdap will be available at the special clinic. No other immunizations will be provided.

Other vaccines will be available at the DOH-Santa Rosa Milton location, 5527 Stewart Street, during regular business hours. The Florida Department of Health offers vaccines to eligible children at no fee from the age of 2 months through 18 years.
