Florida School Requirements
The Immunization Guidelines (PDF-54KB) for the State of Florida cover entry, attendance or transfer to preschools, public/non-public schools (K-12), licensed daycare facilities, and family day care homes. The school the child attends will have on file a Florida Certification of Immunization (also referred to as a "Form 680").
For your convenience, we have provided immunization forms that you can download, print, fill out, and bring in.
